Advanced Medical Directives

Often referred to as AMD's, these documents are invaluable when considering end-of-life decisions. There are many issues for the individual to consider regarding health care and provisions, including those of feeding tubes, resuscitation, and the final rest.

I am sensitive to being of service in these delicate situations, and am as comfortable with bedside signings as with “kitchen-table” signings.

* My Notary Public Commission requires me to notarize based on evidence of your identity. Therefore your valid (current) Driver's License, Passport or California ID card will be required on signing.
